Prizes Under $600All cash prizes under $600 can be redeemed at ANY participating retailers across the state, no claim form is needed. Yes, you can cash in your ticket right away! For a retailer nearest you click here. If mailing your winning ticket to the Lottery Headquarters, please enclose a copy of a photo ID and a completed Claim Form. Prizes Over $600Where to Claim Your Prize Documents You Need
Agreement to Share Ownership and Proceeds of Lottery Ticket All parties sharing the proceeds will need to fill out areas 1 through 11 on the Claim Form #2. All parties will need to provide two legible forms of identification, including a copy of a photo id. If the entire group is not present at the time the claim is filed, the designated group representative will need to provide all of the above mentioned forms and identification information for each individual winner. Applicable tax will be deducted from each share. Unless written authorization is given to group representative, the check will be mailed.
